this form is for people who want to place their orders through the mail. use the pull-down menus to choose your design, shirt and size and type in your choices of color as you browse, then use the print function in your browser to print this page (usually file/print in the menu). or print it as is and fill it in with a pen. if you don't have a printer, just write everything down clearly on a piece of paper. if you want more than 5 shirts, just print this form twice or clearly mark on the printout your additional selections.

please note that we prefer payment in the form of a check or international money order made out to zero t-shirts. we cannot accept credit card payment through the mail at this time. all prices listed here are in US funds and include shipping within north america. international orders please add $3 US. once we receive your order it generally takes 7 to 10 days to get your shirts printed and shipped, so allow at least 3 to 4 weeks for your order to arrive from the day you send it.
